目的  探讨以院外延伸护理小组为基础的肺康复训练在慢性阻塞性肺疾病(COPD)稳定期管理中的应用效果。方法  收集大连市中心医院2018年1月—2019年12月收治的COPD患者40例,随机分为对照组和干预组各20例。两组患者住院期间均接受常规护理,干预组在出院后接受院外延伸护理小组基础上的肺康复训练,训练时间为60 d。比较两组患者的肺功能、圣乔治呼吸问卷和慢性病管理自我效能感量表评分改善情况。结果  干预后,干预组肺功能的FVC、 FEV1和FEV 1/FVC均高于对照组,差异有统计学意义(P<0.05);干预组圣乔治呼吸问卷的症状评分、活动评分、影响评分和总分均低于对照组,差异有统计学意义( P<0.05);干预组慢性病管理自我效能感量表的症状管理自我效能感、疾病共性管理自我效能感和总分均高于对照组,差异有统计学意义(P<0.05)。结论  对于COPD稳定期患者来说,以院外延伸护理小组为基础的肺康复训练能够进一步提高肺功能,减轻健康状况受损程度和改善自我效能,值得临床推广应用。
Objective To explore the application effect of lung rehabilitation training based on out-of-hospital extended nursing team in the stable management of chronic obstructive pulmonary disease (COPD). Methods A total of 40 patients with COPD treated in Dalian Central Hospital from January 2018 to December 2019 were randomly divided into control group and intervention group. Both groups received routine nursing during hospitalization. The intervention group received lung rehabilitation training based on extended nursing team after discharge, and the training time was 60 days. The improvement of pulmonary function, St. George's respiratory questionnaire and chronic disease management self-efficacy scale were compared between the two groups. Results After the intervention, FVC, FEV1 and FEV1/ FVC of pulmonary function in the intervention group were significantly higher than those in the control group (P<0.05). The symptom score, activity score, influence score and total score of St. George's respiratory questionnaire in the intervention group were significantly lower than those in the control group (P<0.05). The symptom management self-efficacy, disease commonness management self-efficacy and total score of chronic disease management self-efficacy scale in the intervention group were significantly higher than those in the control group (P<0.05). Conclusion For patients with stable COPD, lung rehabilitation training based on out-of-hospital extended nursing team can further improve lung function, reduce the degree of damage to health status and improve self-efficacy, which is worthy of clinical application.
