 目的  探索血CA125水平能否预测体外受精 -胚胎移植(IVF -ET)后先兆流产患者的预后,并寻找合适的CA125切点值。方法  回顾性分析2018年7月—2019年3月华中科技大学同济医学院附属同济医院收治的IVF ­ET后先兆流产患者191例,其中继续妊娠组122例,流产组69例。血CA125水平为主要研究指标,采用单因素方差分析、卡方检验、R O C曲线进行统计学分析。结果  流产组患者的血C A125水平高于继续妊娠组,差异有统计学意义(P<0.05)。CA125的预测能力较好,其切点值为37.5 U/mL,敏感性为95.9%,特异性为60.1%,Youden指数为0.56,ROC曲线下面积为0.86(95% CI:0.81~0.92)。结论  CA125可有效预测IVF-ET后先兆流产患者的预后,CA125高提示流产的风险高,其切点值为37.5 U/mL。但临床应用中应注意其特异性相对较低。
 Objective  To explore whether the serum CA125 level can predict the prognosis of patients with threatened abortion after IVF-ET, and to find a suitable cut off value of CA125. Methods  A retrospective analysis of 191 cases of IVF-ET threatened abortion admitted to Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of science and technology from July 2018 to March 2019 was conducted, including 122 cases of continuous pregnancy and 69 cases of abortion. The serum CA125 level was the main research index, and statistical analysis was carried out by ANOVA, chi square test and ROC curve. Results  The level of CA125 in the abortion group was significantly higher than that in the continuous pregnancy group (P<0.05). CA125 has a good prediction ability. The 37.5 U/mL was set as cutoff value,, the sensitivity is 95.9%, specificity is 60.1%, Youden index is 0.56, and area under ROC curve is 0.86 (95% CI: 0.81~0.92). Conclusion  CA125 can effectively predict the prognosis of patients with threatened abortion after IVF-ET, and the high CA125 indicates the high risk of abortion, with a cut-off value of 37.5 U/mL. However, it should be noted that its specificity is relatively low in clinical application.
