 目的  分析在因瘢痕子宫或巨大胎儿行剖宫产产妇中,应用缩宫素联合麦角新碱预防宫缩乏力产后出血的效果。方法  选取2017年10月—2018年12月泗阳县中医院接收的因瘢痕子宫或巨大胎儿行剖宫产产妇102例为研究对象,随机分为对照组和观察组各51例。对照组产后静脉注射缩宫素,观察组静脉注射缩宫素联合肌内注射麦角新碱,对比两组产后2 h、24 h出血量以及红细胞下降值、血红蛋白下降值、宫缩乏力产后出血发生率及不良反应发生情况。结果  观察组产后2 h、24 h出血量以及红细胞下降值、血红蛋白下降值、宫缩乏力产后出血发生率均低于对照组,差异均有统计学意义(P<0.05)。观察组不良反应发生率略高于对照组,但差异无统计学意义(P>0.05)。结论  缩宫素联合麦角新碱能够减少因瘢痕子宫或巨大胎儿行剖宫产产妇的产后出血量,预防宫缩乏力产后出血,且不会增加不良反应,值得推广使用。
 Objective  To analyze the effect of oxytocin combined with ergonovine in preventing postpartum hemorrhage due to uterine atony. Methods  From October 2017 to December 2018, 102 cases of cesarean section women with scarred uterus or fetal macrosomia, which were collected from Siyang Hospital of Traditional Chinese Medicine, were selected as the research objects. According to the random number table, all women were divided into the control group and the observation group, 51 cases in each group. The control group was given intravenous oxytocin, while the observation group was given intravenous oxytocin combined with ergometrine intramuscularly. The amount of bleeding, the decrease of erythrocyte, the decrease of hemoglobin, the incidence of postpartum hemorrhage and the occurrence of adverse reactions were compared at 2 and 24 hours after delivery. Results  The blood loss, erythrocyte decrease and hemoglobin decrease in the observation group were significantly lower than those in the control group at 2 h and 24 h after delivery (P<0.05).The incidence of postpartum hemorrhage in the observation group (5.88%) was significantly lower than that in the control group (21.57%) (P<0.05). The incidence of adverse reactions in the observation group (5.88%) was slightly higher than that in the control group (1.96%) (P>0.05). Conclusion  Oxytocin combined with ergonosine can reduce the amount of postpartum hemorrhage, and prevent postpartum hemorrhage of uterine atony, and do not increase adverse reactions, so it has the value of promotion and application.
