 目的  分析肺癌合并2型糖尿病患者的临床特点。方法  对2017年7月—2019年6月在辽宁省人民医院住院的450例肺癌患者的临床资料进行回顾性分析,所有患者依据是否患有2型糖尿病进行分组研究,比较两组患者的一般资料、凝血功能和肿瘤标记物水平。结果  合并2型糖尿病的肺癌患者(观察组)其年龄、体质量指数、糖尿病家族史与未合并2型糖尿病的肺癌患者(对照组)比较,差异均有统计学意义( P<0.05)。两组性别、吸烟史差异无统计学意义(P>0.05)。两组肺癌病理分型差异无统计学意义(P>0.05)。两组凝血酶原时间、活化部分凝血活酶时间差异有统计学意义(P<0.05),其他凝血功能指标差异无统计学意义(P>0.05)。两组癌胚抗原、神经元特异性烯醇化酶和细胞角蛋白19片段水平差异有统计学意义( P<0.05)。结论  肺癌与2型糖尿病能够相互影响,并相互促进两种疾病的发生和发展。对于高龄、高体质量指数、有糖尿病家族史的肺癌患者,要高度警惕其罹患2型糖尿病的可能。
Objective To analyze the clinical characteristics of lung cancer with type 2 diabetes mellitus. Methods  The clinical data of 450 patients with lung cancer hospitalized in Liaoning Provincial People’s Hospital from July 2017 to June 2019 were retrospectively analyzed. All patients were divided into two groups according to whether they had type 2 diabetes mellitus or not. The general data, coagulation function and the level of tumor markers were compared between the two groups. Results  There were significant differences in age, body mass index, family history of diabetes mellitus between lung cancer patients with type 2 diabetes mellitus(observation group) and those without type 2 diabetes mellitus(control group)(P<0.05). There were no significant differences in gender and smoking history between the two groups(P>0.05). There was no significant difference in pathological classification of lung cancer between the two groups(P>0.05). There were significant differences in prothrombin time and activated partial thromboplastin time between the two groups(P<0.05), but no significant differences in other coagulation function indexes(P>0.05). The levels of carcinoembryonic antigen(CEA), neuron specific enolase(NSE) and cytokeratin 19 fragment were significantly different between the two groups(P<0.05). Conclusion  Lung cancer and type 2 diabetes can interact and promote the occurrence and development of the two diseases. For elderly lung cancer patients, patients with high BMI, and patients with diabetes mellitus family history, the possibility of type 2 diabetes mellitus should be highly vigilant.
